Camp Ma-Ka-Ja-Wan, Wisconsin
We'll be camping at Camp MA-KA-JA-WAN (map). We've planned an action-packed adventure featuring canoeing, shooting sports, rock climbing, ziplining, swimming, and more! Don't miss out on this exciting opportunity to make memories, forge friendships, and experience the great outdoors like never before.
Young Men Registration
Each young man needs to have a permission slip signed by parents and a medical form signed by a physician to be cleared for camp. You can find a blank Permission and Medical release form here: (Permission Form, Medical release)
Important Note: Camp Ma-Ka-Ja-Wan requires the medical form linked above. You'll need to complete the form in it's entirety (including the section that will need to be completed by a physician) and return that form to a member of your Bishopric before going to camp. You will not be allowed to stay at camp without this completed form. The permission slip will be uploaded as part of the registration.
